근육의 모터 포인트 (Muscle motor point)

근육의 Motor point의 정확한 정의는 무엇일까?

"Motor points are elecrophysilogically defined, as the point with the highest excitability of the muscle or the point on the skin where muscle contraction can be observed by the least electrical stimulation or the point on the muscle where muscle contractions can be caused by the minimum intensity and short duration electrical stimulation. Which, anatomically, is defined as the area where motor endplates (terminal area of the motor nerve fiber) are dense."

즉, 모터포인트란 최소한의 전기적 자극으로 최대의 근육 활성도를 일으킬 수 있는 부위를 말한다.

해부학적으로 Motor endplate가 밀집된 부분을 뜻하게 되는데, Motor endplate는 신경자극이 근육의 활성을 일으키는 신경 말단부를 뜻한다.

위의 그림과 같이 Alpha motor neuron에서 시작된 전기적 신호는 Axon을 타고 motor의 endplate에 도착하게 되고, 이렇게 도착한 전기적 신호는 Motor endplate로 부터 근섬유 (Muscle fiber)를 따라 좌, 우로 전달되게 되며 이러한 전기적 신호로 인해 근육에서는 기계적 수축이 일어나게 된다.

Motor point가 유의미하게 작용하는 경우는 우리가 근전도 검사 (Needle EMG)를 진행할 때 근육의 Motor point를 확인해야 조금 더 정확하고 세밀한 근육의 전기적 반응을 확인할 수 있기 때문이다.

​

병원마다 조금씩 차이가 있으나 대부분의 대학병원에서는 상체에서 5개의 근육, 하체에서 5개의 근육에서 근전도 검사를 시행하게 되고 해당 근육들의 Motor point를 충분히 이해하고 있다면 더욱 정확하고 신뢰도가 높은 근전도 검사를 진행할 수 있을 것이다.

상지 근육 (Upper extremities)

​

1) Biceps brachii

C5, C6 신경의 지배

Musculocutaneous nerve 지배

​

Motor point : Medial crease로 부터 4개 손가락 위에 가장 근육이 통통한 곳에 존재함.

​

2) Pronator teres

C6, C7 신경의 지배

Median nerve 지배

​

Motor point : Biceps tendon 과 Medial epicondyle을 잇는 선 중앙에서 손가락 2개 아래정도 존재함.

3) Extensor digitorum communis

C7, C8 신경의 지배

Radial, Posterior interosseous nerve의 지배

​

Motor point : 손목부터 팔꿈치를 1/3으로 나눈뒤, 위쪽 1/3부분에 엄지손가락을 Ulnar에 중지손가락을 Radius에 대고 중간지점을 두번째손가락으로 가르킨 지점에서 존재 함.

4) First dorsal interosseos

C8, T1 신경의 지배

Ulnar nerve의 지배

Motor point : 두번째 Metacarpal의 radial 쪽에 존재함.

​

5) Abductor pollicis brevis

C8, T1 신경의 지배

Median nerve의 지배

​

Motor point : 손바닥을 volar쪽에서 바라본 뒤 첫번째 Metacarpophalangeal joint와 carpometacarpal joint 사이의 중앙 지점에 존재 함.

하지 근육 (Lower extremities)

​

1) Vastus medialis

L2, L3, L4 신경의 지배

Femoral nerve 지배

​

Motor point : Patella의 sup-medial margin 에서 4개의 손가락 위 부분에 존재 함.

​

2) Tibialis anterior

L4, L5 신경의 지배

Deep peroneal nerve 지배

​

Motor point : Tibial tuberosity 에서 4개 손가락 아래, Tibial crest에서 1개 손가락 Lateral side에 존재 함.

​

3) Peroneus longus

L5, S1, S2 신경의 지배

Superficial peroneal nerve의 지배

​

Motor point : Fibular head에서 3개 손가락 아래에 존재 함.

​

4) Gastrocnemius

S1 신경의 지배

Tibial nerve의 지배

​

Motor point : Popliteal crease에서 5개 손가락 아래, 가장 중앙부위의 통통한 부분에 존재 함.

​

5) Tensor fascia lata

L4, L5, S1 신경의 지배

Sup. gluteal nerve의 지배

​

Motor point : Greater trochanter에서 2개 손가락 아래에 존재 함.
